Use Report Forms B if your grant agreement letter has directed you to these forms below. Instructions for completing the reports can be found on the forms.
For Progress Reports, complete Form B1 and the Expenditure Report Form.
For Final Reports, complete Form B2 and the Expenditure Report Form.
Refer to your grant agreement letter for specific due dates.
You can download and save these forms to your computer. To download a form click on the icon and then choose "Save."
