For Competitive Grants

Those who received Community, Diversity Partnership and Rydell Visual Arts Grants should fill out and submit reports through our Online Grant Portal.

For All Other Grants

Grantees should use of the forms following the instructions below. Please refer to your grant award letter, which specifies reporting requirements, due dates and the correct form to use.

Report Form A

Use Report Form A if your grant agreement letter directed you to this form. Refer to the letter for due dates.

Report Forms B

Use Report Forms B if your grant agreement letter directed you to these forms. Refer to your grant agreement letter for due dates.

Progress reports: complete Form B1 and the Expenditure Report Form.

Final reports: complete Form B2 and the Expenditure Report Form.

Report Form C

Use Report Form C if your gift agreement letter directed you to this form. Refer to the letter for due dates.

  • Make it a learning opportunity. We understand the desire to focus on success, yet we're just as interested in hearing about your challenges. Share what you learned and what you might have done differently.

Submitting the grant report

  • Email is our preferred method for reports.
  • Completed reports should be sent to grants@cfscc.org.
  • Type your grant number in the subject line of the email.
